Advance Care Planning

OLLI at UNC Asheville regularly holds an advance care planning (ACP) workshops. Workshops feature a panel whose members are experienced in addressing end-of-life issues. The discussion will include communicating your treatment wishes to loved ones and to medical personnel, ethical and legal issues, and the uses of advance directives. Ample time will be reserved for questions.

Assistance will be provided for anyone wishing to complete a legally valid advance directive, including the notarization required in North Carolina, the NC ACP “Short Form”. You may also find useful the Advance Health Care Directives section of the website of the North Carolina Secretary of State which includes links to downloadable forms for Health Care Power of Attorney, Advance Directive for a Natural Death (Living Will) and forms to become and Organ/Tissue Donor. Finally, you may find out more at Five Wishes. You may also find useful this pamphlet from the NC Department of Health and Human Services, titled “Medical Care Decisions and Advance Directives: What You Should Know.”
